Philipine EMBASSY INFORMATION(SOURCE PHIL. EMBASSY ,ITAEWON ,SEOUL

The applicant is required to personally file the application. Authorization letters may be issued in case the applicant cannot personally claim the processed documents.
Requirements for Renewal
- Duly accomplished passport application form
- Xerox copy of previous passport (for brown passports:first and second pages and date of last entry to Korea; for green passports: first and last pages and date of last entry to Korea)
- For brown passports, any document with full middle name, e.g. Authenticated Birth Certificate, Voters Affidavit, or Baptismal Certificate
- 3 pictures (Photos should have a background color of royal blue. Size is 4.5 x 3.5 cm.)
Filipinos who wish to renew their passports are advised to do so at least six months before the date of expiration. Old passports need to be presented at the time of application.
Requirements for Lost Passport Applications
- Duly accomplished passport application form
- Duly accomplished Affidavit of Lost or Mutilated Passport
- 3 pictures (Photos should have a background color of royal blue. Size is 4.5 x 3.5 cm.)
- Photocopy of old passport, if available. Otherwise, birth certificate issued by the NSO or the local civil registrar
For lost passports, an additional clearing period of one (1) month is required.
Requirements for Passport Amendments
- Duly accomplished passport application form
- Photocopy of marriage certificate
- Original passport
Requirements for New Applications (i.e.,Filipino babies born in Korea)
- Duly accomplished passport application form
- 3 pictures (Photos should have a background color of royal blue. Size is 4.5 x 3.5 cm.)
- Report of Birth (please see Item Number 2 below for details)
Requirements for Travel Documents
- Duly accomplished Travel Document application form
- Duly accomplished Affidavit of Lost / Mutilated Passport
- Any document identifying the applicant: birth certificate, driver’s license, school ID or, in the absence of the preceding documents, any of the following: entry record from Immigration Office, Xerox copy of passport or Alien Registration Certificate
- Three (3) colored or black & white 2" x 2" pictures
| Requirements for Reports of Birth |
- Birth certificate of the child with seal of the hospital or doctor with the corresponding English translation
- Valid Philippine passport or Travel Document of parents.
- Authenticated Marriage Contract of parents (if applicable)
- Any additional document of identification of the parents: birth certificate, voter’s affidavit, NBI, Driver’s License, etc.
- Affidavit of Acknowledgement of Paternity for child born out of wedlock.
- Pictures (size: 1.77" x 1.37" ? colored with plain, light background)
- For a child who will be returning to the Philippines unaccompanied by any of his/her parents: duly accomplished Affidavit of Parental Consent and photocopy of the passport of the traveling companion.
Submit four (4) copies each of Items 2.a to 2.f. Do not photocopy. The baby does not have to appear at the Embassy as long as his/her footprints are already in the form. If the parents have already renounced their Filipino citizenship at the time of report, the parents must also submit their Birth Certificates and Certificate of Renunciation.

Procedure
- Have the Divorce Decree authenticated by the Ministry of Foreign Affairs and Trade (MOFAT), Korea.
- Bring the MOFAT authenticated Divorce Decree and a copy to the Philippine Embassy for final authentication.
- Bring copy of Marriage Contract
- Accomplish the "Report of Divorce" form of the Embassy
- Copy of passport (Passport (Picture Page)(If amended passport, plus copy of page with amendment) (3 photocopies)
- Birth Certificate issued on security paper by the NSO (Original & 1 photocopy)
- Costs :

- Authentication of MOFAT Document
- Notarial Fee
- Form for Application of Registration and Notification of Divorce

Following are the requirements for Filipino citizens who intend to get married while residing in Korea:
- Certificate of Legal Capacity to Contract Marriage
- Duly accomplished Application to Contract Marriage
- Certificate from the National Statistics Office that the applicant has no record of marriage.
- Birth Certificate authenticated by the National Statistics Office.
- Valid Philippine passport or Travel Document.
- For applicants 18-20 years old, notarized affidavit from parents granting Parental Consent for Marriage.
- For applicants 2l-25 years old, notarized affidavit from parents giving Parental Advise for Marriage.
- Notarized affidavit of singleness executed by both parents.
If both the bride and the groom are Filipino citizens, their marriage can be officiated at the Philippine Embassy by a consular officer.
If either the bride or the groom is NOT a Filipino citizen, then the marriage may be officiated at a local Korean city hall; however, the Filipino citizen must first get a "Certification of Legal Capacity to Contract Marriage" from the Embassy.
Abroad, a "wedding" conducted in the church is only a "blessing" and is not registered in the Philippines; only marriages officiated at the Embassy and at a local Korean city hall are recognized as legal and binding.
Additional documents related to marriages include the Affidavit of Eligibility for Marriage and the Report of Marriage. These will be available at the Embassy.
Filipino citizens are reminded to report their marriages officiated abroad to the Philippine Embassy.

From Banpo Bridge: Drive towards the direction of City Hall / Namsan 3rd Tunnel. Turn right at the Kyeongridan intersection and drive towards the direction of Hyatt Hotel.
