Passport & Travel Documents
The applicant is required to personally file the application. Authorization letters may be issued in case the applicant cannot personally claim the processed documents.
Requirements for Renewal
The Embassy will receive applications for the Machine Readable Passport (MRP). You are advised to apply for early renewal of the old “green” passport.
Each applicant must personally appear at the Consulate, except for minors 8 years old and below and senior citizens 65 years old and above
Requirements:
Step 1:
Have your picture taken. Four (4) identical photos taken within the last six months of size 4.5 cm by 3.5 cm on a royal blue background
Applicant should be in decent attire with collar. Both ears should be shown
One (1) copy of the passport data page and one (1) copy of page with last entry to Korea
Step 2:
Fill up passport application form
Get a number from machine by pressing “Passport”. Wait for your number to be called at Window 4.
Present your most recent expired Philippine Passport
Be ready with exact amount of passport fee - KW 52, 650.
Step 3:
After you have paid and received your receipt. Wait for your name to be called for encoding of the data for the Machine Readable Passport
You will be asked to check the entries. Sign your name and make your thumbmarks.
Keep your receipt and your old passport for cancellation and return to the Embassy on due date to claim your new Machine Readable Passport.
NOTE:
It takes about six (6) weeks to process the MRP as the approved applications are sent to a central processing facility in the Philippines. You can authorize a representative to claim your passport by giving authority at the back of your claim receipt. Have your claim receipt stamped with authority to claim at Window 4.
Requirements for Lost Passport Applications
Duly accomplished passport application form
Duly accomplished Affidavit of Lost or Mutilated Passport
3 pictures (Photos should have a background color of royal blue. Size is 4.5 x 3.5 cm.)
Photocopy of old passport, if available. Otherwise, birth certificate issued by the NSO or the local civil registrar
Note:
For lost passports, an additional clearing period of one (1) month is required.
Requirements for Passport Amendments
Duly accomplished passport application form
Photocopy of marriage certificate
Original passport
Requirements for New Applications (i.e.,Filipino babies born in Korea)
Duly accomplished passport application form
3 pictures (Photos should have a background color of royal blue. Size is 4.5 x 3.5 cm.)
Report of Birth (please see Item Number 2 below for details)
Requirements for Travel Documents
Duly accomplished Travel Document application form
Duly accomplished Affidavit of Lost / Mutilated Passport
Any document identifying the applicant: birth certificate, driver¡¯s license, school ID or, in the absence of the preceding documents, any of the following: entry record from Immigration Office, Xerox copy of passport or Alien Registration Certificate
Three (3) colored or black & white 2" x 2" pictures
